2. Please fill out Club Sports Participation Agreement and bring it and a copy of your health insurance card to your tryout date along with a $70 check made out to the Penn State Baseball Club.

IMPORTANT!: YOU MUST HAVE THE CLUB SPORTS PARTICIPATION FORM ALONG WITH A PHOTOCOPY OF YOUR HEALTH INSURANCE CARD AT THE TRYOUT. YOU WILL NOT BE PERMITTED TO TRYOUT WITHOUT IT!
